Public Housing Waiting List

This housing authority offers five communities for families and senior/disabled individuals.

There are three ways to apply for public housing:

1. Download an application here, then print and complete.

2. Visit the housing authority office to pick up an application, located at 240 East Center St., Provo, UT 84606, Monday-Thursday between the hours of 7:30am and 5:30pm.

3. Complete an application online here.

Once the paper applications have been completed, they can be mailed or hand delivered to the address listed above. Be sure to include copies of these documents with your application: social security cards, birth certificates, proof of all income, bank statements, and photo IDs for all adult household members.
