To apply, visit the LHA office to pick up an application, located at 609 E 3rd St., Lexington, NE 68850, from 9:00 am until 12:00 pm; and 1:00 pm until 4:00 pm CT, Monday-Friday.
Once the application has been completed, it must be hand delivered to the address listed above, from 9:00 am until 12:00 pm; and 1:00 pm until 4:00 pm, Monday-Friday.
Be sure to include these documents with the application: Social Security Cards and birth certificates for all household members, and photo IDs for all adult household members. All adult household members must be present when handing in the application.
